1. Lower cauldron
2. Grind wart and nettles, throw them in
3. 4 bellows
4. Put belladonna into the dish
5. Grind Charcoal and also put it into the dish, throw them in
6. 1 bellow
7. Pour
My alchemy skill is pretty high, so some of the bellows are probably also optional, but I can't be arsed to test
