A single Dishonorable Kill can wipe out 10+ hours of PvP grind.
DKs are shared between an entire raid group, so if you get 40 people in a raid and go try to attack Stormwind or something, if a single player kills a single Civilian NPC, the entire Raid gets a DK each.
So any sort of organized, large-scale, FUN PvP is impossible to do because people simply won't do something that risks tanking their honor grind.
